If selected for the Alpha Core of Excellence (A.C.E.) Youth Development Program, I understand that regular attendance, punctuality, respectful conduct, and active participation are expected. If the applicant cannot attend a program activity, the applicant or parent/guardian will notify program leadership in a timely manner. I also understand that parent or guardian engagement is expected throughout the program year.

Photo and Media Release

I grant permission to the Alpha Onward and Upward Foundation, Inc. and the Delta Iota Lambda Chapter of Alpha Phi Alpha Fraternity, Inc. to use my child's likeness in photographs, video, print materials, websites, and official social media platforms for lawful promotional, educational, and program-related purposes without compensation. I understand that these materials may be edited, published, and distributed by the organizations and will remain their property. I release and hold harmless both organizations from claims arising from the authorized use of these materials.
